Staffed Community Residences

Fidelity House provides opportunities for small group living in 19 homes located throughout the Merrimack Valley. These homes are staffed 24 hours a day and provide a safe and supportive environment as needed by the individual residents. Direct support professionals assist each person in developing skills needed to care for themselves and their home and to participate fully in the life of their community.

Individual Support Services

Fidelity House serves men and women living in their own apartments, alone, with a roommate, or with spouses and children. Staff supports are determined by individual preferences and need. Skill teaching typically includes areas such as money management, health care, menu planning, and parenting. People are supported to explore social and recreational resources in their communities. Individual Support Services is a flexible model that responds to the changing needs of the people served. It includes an emergency on-call system, which provides access to 24-hour staff support.

Fidelity House Human Services is pleased to announce the newest addition to their array of residential services for individuals with disabilities. The agency has received approval to establish its Adult Family Care (AFC) program. Adult Family Care is a MassHealth program that provides compensation to family members or non-family members so that they may care for individuals with developmental disabilities in a home setting. Goals of the program are to delay or prevent out of home placements for individuals who are unable to live alone, and/or to continue to support individuals with changing needs within their agency of choice.

Family members, who are not guardians, may become caregivers for an individual age 16 or older who have MassHealth insurance and due to medical, physical, cognitive, or psychiatric problems cannot safely live alone. Non-family caregivers are also able to provide services in their homes. Caregivers receive tax exempt payment for room and board, training, respite and on-going support from an AFC Care Manager, Registered Nurse and Program Director.

Caregivers

Caregivers provide assistance to individuals with activities of daily living such as; bathing, toileting, dressing, and other elements of personal care; house cleaning, laundry, and shopping. Individuals also are assisted with medication management, managing their health care, and are accompanied to their medical appointments.
Caregivers are provided initial and ongoing training by Fidelity House Nurses, Care Managers and the Program Director.
