Adult Foster Care

Through the Adult Foster Care program, Fidelity House CRC provides a tax-exempt stipend along with supportive services so that a family member or other caregiver can care for an adult with developmental disabilities.

Caregivers provide room and board, assistance with the activities of daily living, and transportation to medical appointments. Fidelity House CRC provides nursing assessments and consultation, safety training, and respite care to support the caregiver. Services are available in Lawrence, Methuen, Haverhill, Georgetown, Andover and across Northeastern Massachusetts. For more information, contact us at 978-685-9471 or afc@fidelityhhs.org

There are 4 prerequisites for the Fidelity House CRC Adult Foster Care Program. See below for details.

Your family member must have a letter of eligibility from the Massachusetts Department of Developmental Services (DDS). For information on becoming DDS eligible, go to the Application for DDS Eligibility.

To qualify for Adult Foster Care, an individual with disabilities must have MassHealth insurance. Fidelity House CRC case managers and nursing staff can assist with the MassHealth application process.

Individuals with disabilities may apply for the Adult Foster Care program starting at age 16.

To qualify for the Adult Foster Care, it must be documented that an individual with disabilities needs some help with the activities of daily living such as bathing, toilet use, dressing, etc. or cannot be left alone for a period longer than three hours.

  • Social distancing is remaining out of congregate settings, avoiding mass gatherings, and maintaining distance (approximately 6 feet or 2 meters) from others when possible.
